What this page collects

The application asks for your contact details, practice information, service area, services, and interest in participating. The shorter interest form collects your email and, if provided, practice name. Please do not submit patient, client, medical, or payment information.

How your information is used

Application details are stored for Kindred’s cohort administrator to review potential pilot fit and contact you about next steps. The application’s optional updates checkbox records your preference; completing the separate email signup and confirmation subscribes you to updates. Signing up through “Keep me in the loop” saves your interest with Kindred and starts the separate email confirmation process.

Email updates through Kit

Kindred uses Kit to manage email subscriptions, confirmation, delivery, and unsubscribe requests. When you complete email signup, Kit receives your email address and optional practice name for those purposes. Your full cohort application remains with this website and is not sent to Kit through the signup forms.

Use the confirmation link in the email from Kindred to finish subscribing. You can stop optional updates through the unsubscribe link in a Kindred update email.

Access and hosting

Submissions are stored with this website’s hosting services and are available through an inbox restricted to the Kindred site owner. They are not displayed publicly or made available to other applicants. The hosting platform may process technical information needed to operate and protect the website. The form uses a short-lived, hashed request identifier to limit repeated submissions.

What submitting means

Submitting an application does not confirm membership, acceptance into a cohort, a payment obligation, or a particular number of referrals. An application receipt on this website confirms that your application was saved. It does not confirm an email subscription; email updates require the separate signup and confirmation.

Updates, corrections, or removal

Use the form below to request a correction or removal, ask about your information, or ask the administrator to stop optional updates. Include the email address you used to sign up. Requests are saved for administrator review; your identity may need to be verified before information is changed.

A request submitted here does not immediately change your email subscription in Kit. To unsubscribe from email updates, use the unsubscribe link in a Kindred update email.
